Photography Consent & Release Form

By signing this form you will allow photos of your child and,

hereby release, discharge, and agree to hold harmless the Moscow United Methodist Church, from any and all liability and claims for accidental personal injury, property damage and expenses incurred, of any nature whatsoever which may be incurred by the child while involved in Vacation Bible School, either at the church facilities or off site locations for any other activities.
